Food Service Applications
- Outdoor dining? If you intend to include outdoor dining in your business plan, you must obtain a separate permit from the Building Department. Find the application and additional information related to COVID procedures here.
- Food Safety Training: You must complete a food safety training program by a state- recognized provider. A copy of your certificate of completion must be provided with your food-service application.
- Develop Menu: You must develop a menu to submit with your New Haven Health Department application.
- Apply! The food-service application requires personal contact information, location of intended business, and business structure information. Make sure you have registered your business with the State before you apply. Apply for a license or permit here. Feel free to reach out to the Food System Policy Division if you need help!
Cost
- Food service establishment license—less than 1,500 sq. ft. of floor area: $150
- Food service establishment license— greater than or equal to 1,500 sq. ft. in floor area but less than 3,000 sq. ft. of floor area: $275
- Food service establishment license—greater than or equal to 3,000 sq. ft. in floor area or has a drive-up window: $475
Additional Information to Consider
- You will need a separate license for more than one establishment operated by the same owner.
- Licenses expire on April 13th of each year and may be renewed upon payment of the annual fee.
- Each license or permit is issued for a period of one year from its issue date
